International Conference on Patient-Oriented Centers for Primary and Long-Term Care

The International Conference on our program "PORT – Patient-Oriented Centers for Primary and Long-Term Care" in Berlin on November 6, 2015, offered a platform to learn more about international examples of regional health centers and the European Forum for Primary Care (EFPC) as well as abroad experiences made in Canada, the US, and Sweden.

In moderated workshops, the participants and the international speakers discussed in depth main aspects of innovative, comprehensive and outstanding health centers for primary care such as multi-professionalism, new models of cooperation and patient-centered care.

The conference also highlighted questions of the fractured in German health care system and underlined the great potential of health centers to meet the primary and long-term care needs in Germany.
